NOP-LP-2D01-01 Davis-Besse CONDITION REPORT NOP-LP2001 -1 D3v003e00 CR Nlumber TITLE: BREAKER HB03 NOT READY FOR RCP 1-2 UNCOUPLED RUN.

DESCRIPTION OF CONDITION and PROBABLE CAUSE (ifknown) Summarize any attachments. Identify what, when, where, why, how.

0 The 13.8 KV Breaker was not installed in the HB03 cubicle ready to support the RCP 1-2 uncoupled run. This caused a delay to the starting time for the RCP 1-2 uncoupled run. The breaker should R have been installed in the HB03 cubicle and ready for racking in to perform the test without delay.

G Note: CRs 03-0042 (Motor rotation wrong way) & CR 03-0049 (Motor tripped) were also generated G for the RCP 1-2 uncoupled run.

I Performed testing on the HB03 breaker to enable installation in the cubicle and racking into the bus.

0 N This condition was not discovered during the Initial attempt to physically run the motor. This condition was determined during the performance of the prerequisites of the test procedure.

Discussion, occurring after the event, has led some to conclude that this condition was discovered at energization of the motor, which Is incorrect.-.

The RCPs are not required to be operable in this Mode, Defueled, I Referred to T.S.3.4.6.1, therefore Operable Is marked N/A. The condition Identified addresses a O timing issue for the breaker being placed inside the breaker cubicle after motor replacement, it does N not address any Issues that would effect breaker operation. Reportable is No.
