ASSESSMENT FOLLOW-UP LETTER-MILLSTONE POWER STATION, UNIT 2
Dear Mr. Christian:
On April 29, 2004, the U.S. Nuclear Regulatory Commission (NRC) staff completed a quarterly review of Millstone Power Station, Unit 2. The assessment evaluated the performance indicators (PIs) and inspection results. The purpose of this letter is to inform you of your safety performance during this period and our plans for a future inspection at your facility.
Our review of Unit 2 first quarter 2004 performance identified that you have crossed the threshold from Green to White for the Unplanned Scrams Per 7000 Critical Hours PI.
Specifically, there were four unplanned scrams since November 2003. Two of the scrams occurred in November 2003, as a result of high turbine generator vibrations on the newly installed low pressure turbine rotors. During the first quarter of 2004, two additional scrams occurred, each caused by the loss of a steam generator feedwater pump. As a result, we have assessed Millstone Power Station Unit 2 performance to be in the Regulatory Response column of the NRCs Action Matrix. Therefore, we plan to conduct a supplemental inspection in accordance with Inspection Procedure 95001 following the completion of your root cause analysis. This inspection procedure is conducted to provide assurance that the root causes and contributing causes of risk significant performance issues are understood, the extent of condition is identified, and the corrective actions are sufficient to prevent recurrence.
Please inform Mr. James Trapp, Chief, Project Branch 6 at 610-337-5186 within 14 days of receipt of this letter of your schedule for completing your evaluation of the issues that led to this White PI. Based on this information, we will schedule our supplemental inspection at a mutually convenient time.
In accordance with 10 CFR 2.390 of the NRC's "Rules of Practice," a copy of this letter will be available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of the NRCs document system (ADAMS).
