SUMMARY
PACKAGES Here's my proposed writeup:
Five green NCVs were issued in the the first quarter of 2003 as result of a fire protection triennial inspection performed in the fourth quarter of 2002. These findings involved: failure to protect MOVs from maloperation due to a fire, manual actions that would not work or presented excessive challenges to the operator, and inadequate emergency lighting in areas with post fire actions. Also the team identified that the licensee has implemented numerous manual actions in lieu of meeting one of the required separation schemes. This is currently being reviewed and will probably result in additional enforcement action.
Further the team noted that the licensee's corrective actions in response to the previously White finding were inadequate.
While no root cause evaluation was performed by the team, both of the triennial inspections performed at the site have revealed shortcomings in the licensee's fire protection program.
