FIELD MONITORING TEAM CHECKLIST Flield Monitoring Team Lead A. Sign in the TSC tag board and get dosimeter/watch.

B. Identify your team member and controller for a drill. -

1. Team consists of a Field Team Lead (Qualified Monitor).
2. Field Team Driver - -

C. Obtain keys to the Gang Box from the Field Team Lead Position Notebook.

D. Inthe Badging Center obtain the Team Gang Box and verify operability of the equipment identified on the appropriate inventory list found in the Gang Box.

E. Check with the HP Supervisor to determine if severe radiological problems exist onsite.

NOTE If a severe radiological problem'dexists onsite, don protective clothing and mask,.as directed by the HP Supiervisor.

F. Obtain a 12-volt battery (automotive type) from the Battery Storage Locker in the Badging Center storage location.

G. Locate the Offsite Field Radiological Monitoring Team'Driver and coordinate briefing.

H. Attend the briefing conducted by the HP Supervisor.

I. Obtain additional protective clothing, respiratory equipment, hi-range dosimetry, etc., as required by the OSC Supervisor.

J. Assist in loading equipment and supplies into the vehicle.

K. HIVOL Sampler.

2. Fill out critique sheet (if necessary)

D. Inthe Badging Center obtain the Team Gang Box and verify operability of the equipment identified on the appropriate inventory list found in the Gang Box.

E. Go to the Security Control Point (SCP) and:

1. Obtain keys for a vehicle, unless already checked out.

F. Obtain 800 MHz hand held radio and magnetic mounted antenna from the TSC G. Check with the HP Supervisor to determine if severe radiological problems exist onsite.

NOTE Ifa severe radiological problem exists onsite, don protective

.,clothing and mask, as directed by the HP Supervisor.

H. Check to see if the vehicle is operable and ifgas tank heeds to be filled.

I. Mount the radio antenna on the vehicle.

J. Obtain a 12-volt battery (automotive type) from 'the Battery Storage Locker in the Badging Center storage location.

K. Locate the Offsite Field Radiological Monitoring Team Leader and advise him that you are ready for the briefing.

L. Attend the briefing conducted by the HP Supervisor.,

M.- Obtain additional protective clothing, respiratory equipment, hi-range dosimetry, etc., as required by the OSC Supervisor. --

N. Assist in loading equipment and supplies into the vehicle.

1. Be very observant of SAFETY even if there appears to be a time crunch
2. Be aware of REAL Road conditions
3. Be aware of REAL Life weather Conditions
4. Safety Checks (set belts..)

T. Communications

1. Monitor Radio traffic and communicate any relevant information to team lead
2. Be able to send and receive radio communications using normal radio protocols
3. Use phonetic alphabet in communicating
4. Use three part communications
5. During Drill use the statement in communications: "This Is a Drill" U. Exercise Completion (Drill or Real)
