One of our reviewers, Gary Hammer, has the following questions regarding your 2/3/03 application for exigent amendment. Please call me to arrange a conference call ASAP to support timely completion of this review. This e-mail does not state a formal NRC staff position, nor does it convey a formal request for additional information. Its sole purpose is to prepare you for the conference call.
- 1. Was vacuum breaker 2ISC*RV36B the only breaker which actuated during the November 2002 S/RV discharge event? If so, why were no others opened since they all have setpoints of
< or = 0.25 psig? Is it possible that, even though the breaker reseated successfully, this event resulted in some damage to the breaker which could affect its current ability to open or reseat?
- 2. Regarding the industry experience with the NMP2 model vacuum breakers, how many of this model are there in the data base? Are there other model breakers which may have relevant operational data? Were there other non-test apparatus failures, prior to 14 years ago? What was the nature of the inadequate maintenance which resulted in the two failures?
