Vermont Yankee 2002 Summary Reports for In-service Inspection and Repairs or Replacements In accordance with Article IWA-6000 of Section XI of the ASME Boiler and Pressure Vessel Code, Vermont Yankee (VY) hereby submits the 2002 Form NIS-I Owner's Summary Report for In-service Inspections and the 2002 Form NIS-2 Owner's Summary Report for Repairs and Replacements.
These reports describe the in-service inspections, repairs and replacements performed during the period May 21, 2001 through October 25, 2002. VY's third ten-year interval began September 1, 1993.

Inservice Discrepancy report # 02-06 was generated to request engineering evaluation. Mechanical Design Engineering dispositioned the IDR as follows: The weld in the as found 3A-N4 condition can perform its design function. The indications were welding/construction related. Remove C-B Athe indications per memo VYM 182/88. Work Order 01-004584-097 was generated to mechanically remove the indications. Ultrasonic thickness readings were taken prior to, during and after removal. The indications were removed with minimal material loss and an acceptable surface examination was documented.
VT-3 visual examination of restraint RSW-H-233 revealed a loose bolt (the nut was tight) on the hanger clamp. Inservice Discrepancy report # 02-01 was generated to request engineering evaluation.
Mechanical Design Engineering dispositioned the IDR as follows: The identified loose bolt does not F-A RSW-H-233 compromise the structural integrity of the clamp/support configuration. The fact that the nut is tight ensures that the bolt will not fall out. The second bolt is actually the load carrying component with no identified anomalies. No further action is required/use-as-is.
VT-3 visual examination of restraint RSW-226A revealed a cotter pin not sufficiently bent. Inservice Discrepancy report # 02-02 was generated to request engineering evaluation. Mechanical Design F-A RSW-226A Engineering dispositioned the IDR as follows: Support function is not degraded by identified condition.
Initiate work order to bend pin to desired configuration." The work was performed as part of the Fix It Now team effort and an acceptable VT-3 was performed to document the work having been performed.
VT-1 visual examination of RHR Service Water Pump H-P-8-1A integral attachment weld revealed numerous areas of under fill and undercut. Inservice Discrepancy report # 02-01 was generated to request engineering evaluation. Mechanical Design Engineering dispositioned the condition via F-A H-P-8-1A Technical Evaluation 2002-042 as follows: A conservative minimum size for the pump barrel weld is a 3/16" fillet weld. Actual field inspection demonstrates that the as found weld configuration/size exceeds the minimum required. The existing weld is therefore demonstrated as capable of transferring/resisting the applied design loading and no further action is required - use-as-is

VT-3 visual examination of spring hanger CS-HD6IB revealed spring can settings outside the acceptable range (+/- 5%). Inservice Discrepancy report # 02-05 was generated to request engineering evaluation of this condition. Mechanical Design Engineering dispositioned the IDR as follows: All values are within the working range of the spring, therefore the support can perform its design function.
F-AUG CS-HD61B The springs are to be reset to the values shown on drawing 5920-6401 sheet 1. The adjacent gravity hangers CS-HD6IC & CS-HD6IA shall be inspected when resetting the springs to ensure that no abnormal pipe movement is observed and that they still function as designed. Work Order 0 1-004584 096 was generated to reset the spring cans to the required setting. An acceptable VT-3 examination was performed after the resetting of the springs.
