SPECIAL REPORT FIRE PROTECTION SYSTEM IMPAIRMENTS REQUIRING NRC NOTIFICATION PURPOSE Technical Specification (TS) 3.14.5.b requires the submission of a report to the NRC if a non-functional fire barrier penetration is not returned to functional status within 7. days.

TS 3.14 also states that certain other conditions require NRC notification.

In accordance with the above requirements, this special report is being submitted in order to notify the NRC.

EVENT Fire Protection Impairment (FPI) 2337 was opened on barrier 093C.

FPI 2347 is on detector alarm point 29.

FPI 2348 was opened on thermolag barrier B-3K1811..

FPI 2365 was opened on thermolag barrier 4F141.

.FPIs 2379 thru 2383, 2385 thru 2387, 2390 thru 2440 and 2443 thru 2446 were opened for not meeting acceptance criteria

'uring the performance of the semi-annual fire door surveillance.

See the attached list.

CAUSE AND CORRECTIVE ACTIONS FPI 2337, opened March 12, 1989 to repair a cracked barrier, was closed on March 21, 1989.

2347, opened March 16, 1989 -for welding in area, was closed on April 4, 1989.

FPI

2348, opened March 16, 1989 for thermolag repair, was closed on March 23, 1989.

FPI 2365, opened March 27, 1989 to remove a pump, is still being worked.

Engineering is presently evaluating the discrepancies identified by the fire door surveillance.

The plant continues to implement the compensatory measures as applicable.

2311, reported open in FPL letter L-89-130, dated April 4, 1989,. was closed on March 23, 1989.

FPI 2204, reported open in FPL letter L-89-012, dated January 9, 1989, was closed March 30, 1989.

FPI 2257, reported open in FPL -letter L-89-58, dated February 23,

1989, was closed on April 1, 1989.

FPI 2297, reported open in FPL letter L-89-114, dated. March 23, 1989, was closed on April 4, 1989.
