SPECIAL REPORT APRIL 4 I 1989 FIRE PROTECTION SYSTEM IMPAIRMENTS REQUIRING NRC NOTIFICATION PURPOSE:

Technical Specification (TS) 3.14.5.b requires the submission of a report to the NRC if a non-functional fire barrier penetration is not returned to functional status within 7 days.

TS 3.14 also states that certain other conditions require NRC notification.

In accordance with the above requirements, this special report is being generated in order to notify the NRC.

EVENT:

Fire Protection Impairment(FPI)

2311, opened 02/28/89, is for Detector Alarm Point 29.

FPIs 2313 thru 2318, opened 03/02/89, are for fire door breaches; FPI 2313 for 67-1, 2314 for 96-1, 2315 for 94-1, 2316 for 68-1, 2317 for 70-1 and 2318 for 71-1.

FPIs 2319 and

2320, opened 03/04/89, were for fire. doors 104-1 and 104-2.

FPI 2326, opened 03/07/89, is on Flamastic Barrier T4J41 Port 2.

CAUSE AND CORRECTIVE ACTIONS:

FPI 2311 is on an alarm that will not reset; work is ongoing.

FPIs 2313 thru 2318 are for fire doors found to be impaired by security grates fastened to the doors.

New fire doors are being purchased.

FPIs 2319 and 2320 were on fire doors left open for ventilation due to equipment breakdown.

FPIs 2319 and 2320 were closed on 03/12/89.

FPI 2326 is for flamastic removal due to I&C testing; work is ongoing.

The plant continues to implement the compensatory measures as applicable.

STATUS UPDATE:

FPI 2182, reported as open in FPL letter L-89-012, dated January 9,

1989, was closed on March 13, 1989.

FPI 2304, reported as open in FPL letter L-89-114, dated March 23,

1989, was closed on March 13, 1989.
