SPECIAL REPORT JANUARY 9, 1989 FIRE PROTECTION SYSTEM IMPAIRMENTS REQUIRING NRC NOTIFICATION PURPOSE: 1 Technical, Specification (TS) 3.14.5.b requires the submission of a report to the NRC if a non-functional fire barrier penetration is not returned to functional status within 7 days. TS 3.14 also states that certain other conditions require NRC notification. In accordance with the above requirements, this special report is being generated in order to notify the NRC. This report is being updated on a monthly basis. EVENT'urkey Point has been and is continuing to implement plant modifications. Fire Protection System (FPS) impairments are, at times, incurred during the performance of these modifications and during plant maintenance activities. Occasionally, certain FPS features fail due to random component failure. CAUSE AND CORRECTIVE ACTIONS: Fire Protection Impairments (FPI) 2173 and 2174'ere opened due to leaching in the Thermolag insulation'. FPI 2162 was opened due to a grout void. FPI 2181 was opened due to a faulty latching mechanism in a fire door, and FPI 2184 was opened due to 'a faulty locking mechanism on another fire door. FPI 2185 was opened in support of ICW pump foundation work. FPIs 2172 and 2190 were opened due to work on the reactor head leak detection system. FPI 2204 was opened to repair spalled concrete. FPI 2195 was opened in support of work on a source range detector. FPI 2182 was opened in support of the Component Cooling Water Heat Exchanger Replacement work. FPI 2142 was opened to replace a regulator. The plant continues to implement the compensatory measures as applicable.
