NOTICE OF VIOLATION Illinois Power Company License No. NPF-62 As a result of the inspection conducted on October 14 through November 25, 1987, and in accordance with the " General Policy and Procedure for NRC Enforcement Actions",10CFR Part 2, Appendix C (1985), the following violation was identified:

Technical Specification 3.0.4 states, in part, that entry into an OPERATIONAL CONDITION or other specified condition shall not be made unless the conditions for the Limiting Condition for Operation are met without reliance on p-cvisions contained in the ACTION requirement Contrary to the above:

On November 18, 1987, while in OPERATIONAL CCNDITION 4, the root valve for the Division I Reactor Vessel Pressure - Low (LPCI and LPCS Injectiun Valve Permissive) instrument was closed during a surveillance and not reopened. On November 21, 1987, the plant entered OPERATIONAL CONDITION 2 with the operators unaware of the isolated pressure instrumen Technical Specification 3.3.3, Teble 3.3.3-1, Item A.1.c, for OPERATIONAL CONDITION 2 (applicable Operational Condition), ACTION 32, required that with the number of OPERABLE channels less than required by the minimum OPERABLE channels per trip function requirement, (4 channels) the associated ECCS be declared inoperabl With Division I of LPCI and LPCS ECCS functions inoperable, entry into OPERATIONAL CONDITION 2 was in violation of Technical Specification 3. This is a Severity Level IV violation (Supplemant I) (461/87036-02(DRP)).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or -
