ENCLOSURE 1 NOTICE OF VIOLATION Florida Power Corporation Docket No. 50-302 ;

Crystal River Unit 3 License No. DPR-72 During the Nuclear Regulatory Commission (NRC) inspection conducted on October 10 - November 12, 1987, violations of NRC requirements were identifie The violations involved a failure to adhere to procedures and a failure to have the proper equipment operating during fuel handling operation In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violations are listed below: TS 6.8.1.s requires the implementation of written procedures for those activities recommended in Appendix "A" of Regulatory Guide 1.33, November 197 Regulatory Guide 1.33, Appendix "A",Section I recommends both general and specific procedures for performing maintenanc Compliance procedure CP-118, Fi re Prevention Work Permit Procedure, requires, in part, in step 4.2.2 that the fire watch will verify that all combustibles have been protected, that floor and wall openings have been protected, and that a fire watch be assigned to watch for sparks in floor areas below where the work is taking plac Maintenance Procedure MP-109, OTSG Relief Valve Maintenance, requires the upper ring settings of the relief valves to be set at +165 (+170 for valves MSV-40 and MSV-48) and the lower ring settings of the relief valves to be set at - Contrary to the above: On October 25, 1987, at approximately 1:15 PM, procedure CP-118 was not implemented in that combustibles were not protected, floor and wall openings were not protected, and a fire watch was not assigned to watch for sparks in lower floor areas. As the result of this implementation failure, welding activities on the 119 foot elevation in the reactor building caused sparks which fell through a floor opening and resulted in a fire in unprotected combustibles on the 95 foot elevation of the buildin . On November 2, 1987, it was determined that procedure MP-109 was not implemented in that the upper and lower ring settings were set in the range of +142 to +174 and -3 to -12 respectively on 14 of the 16 relief valve This is a Severity Level IV Violation (Supplement I).

l Florida Power Corporation 2 Docket No. 50-302 Crystal River Unit 3 License No. DPR-72 TS 6.9.12 requires that the auxiliary building ventilation exhaust fans be operating whenever che pair of exhaust fans are inoperable and fuel is i being moved within the storage poo Contrary to the above, on November 11, 1987, during the period of 9:55 p.m..to 10:04 p.m, one pair of exhaust fans were inoperable and the other pair of exhaust fans were secured while fuel was being moved in the storage poo This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Florida Power Corporation is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including:

-(1) admission or denial of the alleged violations; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
