CLINTON STATION NRC INITIAL LICENSE EXAMINATION REPORT 05000461/2003301(DRS)
Dear Mr. Skolds:
On January 15, 2004, NRC examiners completed initial operator licensing examinations at your Clinton Station. The enclosed report presents the results of the examination.
The NRC examiners administered an initial license examination operating test during the weeks of January 5, 2004, and January 12, 2004. A written examination was administered by Clinton Station training personnel on January 15, 2004. Four Reactor Operator (RO) and six Senior Reactor Operator (SRO) applicants were administered license examinations. The results of the examinations were finalized on February 4, 2005. All applicants passed all sections of their respective examinations and were issued applicable operator licenses.
In accordance with 10 CFR Part 2.790 of the NRCs "Rules of Practice," a copy of this letter and its enclosures will be available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of NRCs document system (ADAMS). SUMMARY OF FINDINGS

Examination Report.
The announced operator licensing initial examination was conducted by regional NRC examiners in accordance with the guidance of NUREG-1021, Operator Licensing Examination Standards for Power Reactors, Revision 9, Draft.
Examination Summary:
Ten applicants (four Reactor Operators (ROs) and six Senior Reactor Operators (SROs)) passed all sections of their respective examinations and were issued applicable operator licenses (Section 4OA5.1).

.1 Initial Licensing Examinations
a. Examination Scope
The NRC examiners conducted an announced operator licensing initial examination during the weeks of January 5 and January 12, 2004. The facilitys training staff used the guidance established in NUREG-1021, Operator Licensing Examination Standards for Power Reactors, Revision 9, Draft, to prepare the examination outline and to develop the written examination and operating test. The NRC examiners administered the operating test during the weeks of January 5 and January 12, 2004. Clinton Station training staff members administered the written examination on January 15, 2004. Four Reactor Operator and six Senior Reactor Operator applicants were examined.
b. Findings
Written Examination The licensee developed the written examination. During their initial review, the examiners determined that the examination, as submitted by the licensee, was within the range of acceptability expected for a proposed examination. During examination validation the week of December 1, 2003, examination changes agreed upon between the NRC and the licensee were incorporated according to the guidance contained in NUREG-1021.
All applicants passed the written examination. The licensee submitted one post examination comment to the written examination. The examiners accepted the requested change. The changes resulting from the examiners post-examination review are documented in Enclosure 2, Post Examination Comments and Resolution.
Operating Test The NRC examiners determined that the operating test, as originally submitted by the licensee, was within the range of acceptability expected for a proposed examination.
Examination changes, agreed upon between the NRC and the licensee, were made according to NUREG-1021.
Examination Results All applicants passed all sections of their respective operating tests. The applicants were issued applicable operator licenses.
.2 Examination Security
a. Examination Scope
The examiners reviewed and observed the licensees implementation of examination security requirements during the examination preparation and administration.
b. Findings
No findings were noted in this area. The licensee staff was observed to be enforcing correct examination security procedures.

.1 The chief examiner presented the examination teams preliminary observations and
findings on January 15, 2004, to Mr. Bement and other members of the Clinton Station Operations and Training Department staff. The licensee acknowledged the observations and findings presented.

POST EXAMINATION COMMENTS AND RESOLUTION
Written Examination Question #78 on the Senior Reactor Operator (SRO) Examination:
This question asked the applicant to assess plant conditions during a postulated loss of steam
to the main turbine seals, and then to select the appropriate procedure for responding to this
abnormal situation.
Facility Comment:
The facility licensee recommended that the correct answer to SRO Question #78 be changed
from, A: Break condenser vacuum..., to B: Use Main Steam to supply the steam seal
header....
The licensee referenced procedure CPS No. 3107.01 GLAND SEAL 8.1.4, INFREQUENT
OPERATION, Operating Without the Gland Steam Seal Evaporator, as justification for the
change.
Resolution:
During examination validation, procedure CPS 3112.01 CONDENSER VACUUM (CA) was
used to justify breaking condenser vacuum as the correct answer. CPS 3112.01 states,
During shutdown, if gland sealing steam is lost, condenser vacuum must be broken
immediately to prevent drawing cool air across the seals. However, from the plant conditions
presented in the stem of this question, it is clear that Gland Steam could be supplied from Main
Steam. Under the conditions presented in the question stem, a shutdown in progress with the
main condenser being used as the heat sink for the reactor, CPS No. 3107.01 GLAND SEAL
8.1.4, INFREQUENT OPERATION becomes the governing procedure during a of loss gland
sealing steam. CPS No. 3107.01 directs that the Main Steam Supply be placed in service to
maintain condenser vacuum. Therefore, the correct answer should have been B: Use Main
Steam to supply the steam seal header..., and the licensees recommendation was accepted.

The following documents observations made by the NRC examination team during the initial
operator license examination. These observations do not constitute audit or inspection findings
and are not, without further verification and review, indicative of non-compliance with 10 CFR 55.45(b). These observations do not affect NRC certification or approval of the simulation
facility other than to provide information which may be used in future evaluations. No licensee
action is required in response to these observations.
During the conduct of the simulator portion of the operating tests, the following items were
observed:
ITEM
DESCRIPTION
1.
During Scenario 1 a rod drift alarm came in during an event dealing
with an uncoupled control rod. This appears to be a fidelity issue in
that the simulator staff believed there should not have been a rod drift
alarm for the simulated plant conditions.
2.
During Scenario 2 Standby Liquid Control pump was scripted to fail to
start. The pump in fact started even though the failure was
programmed into the simulator software.
3.
During the validation week and during the operating examination all the
lights on the reactor control panel would periodically flash on. The
simulator staff reported that this was a long standing problem with the
panel power supply.
4.
During the running of Scenario 2 for the last time, all the valve position
indicator lights for an approximately one square foot section of the Low
Pressure Coolant Injection panel went out. Upon investigation the
simulator staff found a loose wire that they repaired.
