We stated that we would perform a root cause analysis for the events cited. In the-violation, and for subsequent events, to determine appropriate e additional corrective actions, and would submit it to you by July 11,1997.

- A Common Cause Analysis, which investi9 sted personnel performance in the area of radiation worker practices, has been completed. Subsequent to Notice of Violation 336/97 01-09, we received a Notice of Violation applicable to Millstone Unit Nos.1,2,

-and 3.- The root causes and corrective action recommendations contained in the Common Cause Analysis will be utilized in our response to Notice of Violation (245/97--

0217,336/97 02-17, and 423/97 0217),

The revised commitment is as follows:
