KEWAUNEE POWER STATION, UNIT 1 NRC INITIAL RETAKE LICENSE EXAMINATION REPORT 05000305/2009302(DRS)
Dear Mr. Heacock:
On August 7, 2009, the U. S. Nuclear Regulatory Commission (NRC) examiner completed initial retake operator licensing process for an examination at your Kewaunee Power Station. The enclosed report documents the results of the examination. Preliminary observations and findings were discussed on August 7, 2009, with Mr. Terry Evans and members of your staff during an exit meeting with Mr. R. K. Walton of NRC Region III Operator Licensing.
The NRC examiner administered an initial retake license examination operating (Administrative Job Performance Measure) test during the week of August 3, 2009. The written examination was previously administered by Kewaunee Power Station training department personnel on April 23, 2009. The operating examination was previously administered by the NRC during the weeks of April 13 and April 20, 2009. The results of the operating retake examination were finalized on August 27, 2009. The applicant has now passed all sections of the examinations resulting in the issuance of a Senior Reactor Operator license.

The announced initial operator licensing retake examination was conducted by regional NRC examiners in accordance with the guidance of NUREG-1021, Operator Licensing Examination Standards for Power Reactors, Revision 9, Supplement 1.

.1 Initial Licensing Retake Examination
a. Examination Scope
Kewaunee Power Station training staff used the guidance prescribed in NUREG-1021, AOperator Licensing Examination Standards for Power Reactors,@ Revision 9, Supplement 1, to prepare the Administrative Job Performance Measures (JPMs) retake examination outline and the operating test (Administrative JPMs). The NRC examiners validated the proposed examination during the week of July 27, 2009, at the Kewaunee Power Station simulator building with the assistance of members of the licensee training staff. The NRC examiners previously conducted the operating portion of the initial license examination during the weeks of April 13 and April 20, 2009. The NRC Chief Examiner and members of the Kewaunee Power Station training department staff previously administered the written examination on April 23, 2009. The NRC examiners used the guidance established in NUREG-1021, to prepare, validate, revise, administer, and grade the examination.
b. Findings
Operating Test
During the validation of the operating test, minor changes were made to the Administrative JPMs. All changes made to the operating test were made and documented in accordance with NUREG-1021, Revision 9, Supplement 1. The NRC examiner determined that the submitted operating test met the NRCs expectations for an initial license operating test. The NRC examiners completed operating test grading on August 11, 2009.
Examination Results
The applicant was administered a Senior Reactor Operator (SRO) level Operating Exam (Administrative JPMs). The applicant was previously administered an operating test during the weeks of April 13 and April 20, 2009. The applicant was previously administered a written exam on May 7, 2009. The applicant previously passed portions of the operating exam and written test. The applicant successfully passed the remaining portion of the operating test (Administrative JPMs) and was issued an SRO operator license.
.2 Examination Security
a. Scope
The NRC examiners reviewed and observed the licensee's implementation of examination security requirements during the examination validation and administration to assure compliance with 10 CFR 55.49, AIntegrity of Examinations and Tests.@ The examiners used the guidelines provided in NUREG 1021, "Operator Licensing Examination Standards for Power Reactors,@ to determine acceptability of the licensee=s examination security activities.
b. Findings
On July 2, 2009, the licensee documented on Condition Report CR340205 a potential exam security issue. The licensee discovered that the simulator computer copied and retained in memory the last executable malfunction from the executable file on the computer T:\\drive. Since JPM A.5 was the last file run on the simulator and the simulator was released for access to personnel not on the security agreement, JPM A.5 was considered by the licensee to be potentially compromised.
The licensees extent of condition determined that the use of executable files have existed during the last initial license exam and during the last requalification exam. The licensee was unaware of the T:\\drive remembering the last executable malfunction and simulator security checklists did not erase the T:\\drive prior to releasing the simulator.
The examiner reviewed this condition and determined that it was unlikely that anybody knew that the simulator retained the last executable malfunction on the T:\\drive.
Additionally, the examiner determined that it was unlikely that anybody could recreate exam material (JPMs or scenarios) from the last executable malfunction.
Corrective actions included replacing JPM A.5 and implementing instructions to run an ordinary executable file after exam material was administered on the simulator prior to releasing the simulator. The examiner verified proper implementation of the corrective actions on the licensees simulator security checklist.
The issue was reviewed and assessed for a possible violation of 10 CFR 55.49. Based on the above evaluations it was determined that no actual examination compromise had occurred. However, to assure and enhance examination integrity, the JPM was replaced. This issue constitutes a violation of minor significance that is not subject to enforcement action in accordance with the NRCs Enforcement Policy.
4OA6 Meetings
Exit Meeting
The chief examiner conducted an exit meeting on August 7, 2009, with Mr. T. Evans, Kewaunee Power Station Training Manager and other members of your staff. The NRC=s preliminary operating examination results were provided to Mr. Evans during the discussion. The examiners asked the licensee whether any of the material used to develop or administer the examination should be considered proprietary. Proprietary or sensitive information was identified during the examination and is to be withheld from the public document room.
