quantities greater than amounts listed in Appendix C to 10 CFR 20.1001-20.2401 have been used, possessed, or stored.

. Onsite areas, other than restricted areas, where spills or other unusual

occurrences involving the spread of contamination in and around the facility, equipment, or site have occurred that required reporting pursuant to 10 CFR 30.50(b)(1) or (b)(4), including areas where subsequent cleanup procedures have removed the contamination.

Specific locations and radionuclide contents of previous and current burial

areas within the site, excluding radioactive material with half-lives of 10 days or less, depleted uranium used only for shielding or as penetrators in unused munitions, or sealed sources authorized for use at temporary job sites.

Location and description of all contaminated equipment involved in licensed

operations that is to remain onsite after license termination.

7.

Submit a complete renewal application with proper fee, or termination request'at least 30 days before the expiration date on your license. You will receive a reminder notice approximately 90 days before the expiration date. Possession of radioactive material after your license expires is a violation of NRC regulations.

8.

Request termination of your license if you plan to permanently discontinue activities involving radioactive material.

You will be periodically inspected by NRC. Failure to conduct your program in accordance with NRC regulations, license conditions, and representations made in your license application and supplemental correspondence with NRC will result in enforcement action against you. '1he Pharmacy will be located in a one level wood building located in a medical neighborhood.

The generallsyout and site of the building nay be seen in attachment,9.1,9.2,and 9.3.

Restricted areas are outlined. Shielding and distance from regularly occupied work areas - are employed to ensintain exposures in accordance with the ALARA program. A separate lock system will be employed for the restricted area entrances to insure against unauthorized entry by non-radiation workers. All carriers will be instructed in the proper procedures for deliveries.

Secadty Security of radioactive material us nmintained to prevent unauthorized access when facility is closed by locking all entrances / nits when the plannecy is not attended. The building is equipped whh an intrusion alarm system connected to all operating entrances /exhs of the buikling and motion detector.' NON RESTRICTED AREA

  • Ihe controlled area will cond2 of the rest of the pharmacy. All exits from the restricted areas into the non-restricted areas will be equipped with a hand and foot nxmitors, which will be used by personnel to check for contamination prior to exiting the restricted area.

Shielding , In accordance with the ALARA program, sulucient shleiding will be provided to keep the average whole body exposure rate in continuously occupied spaces at or telow 10 mrem per week. The " average" exposure rate shall be measured at not less than 0.5 meters from any radiation source. Areas which have low occupancy factors may be allowed to have higher exposure rates and willbe posted as radiation areas, but adjacent high occupancy areas shall be shickled as required.

Radiopharmaceuticals are stored in lead containers. This shickling is sufDcient to satisfy ALARA criteria. Additional shielding is used as necessary to reduce whole body exposure rates near stored noterials.

RESTRICTED AREA A. Generator Mo99/Ic99 generators in active service will be stored away form any side wall that may be frequented by any non-radiation worker. Each unit will be placed in the auxihary shielding supplied by the manufacturer, and are kept in a lead generator box.

Additional shielding will be used as necessary to satisfy ALARA criteria. Old generators may be transferred to a segregated waste storage area after expiration.

B. Waste Storage - The waste storage bins are constructed of 1/4" lead and have the capacity of approximately 50 gallons. The pharmacy will have 3 of these bins. Additioral shielding will be used as necessary to satisfy the ALARA criteria.

C. Preparation and Dispensing area - -Lead (1/2") and lead glass (1/4") dispensing stations are provided for the preparation, assay, and dispensing of radioactive materials.

Each dispersing area will be equipped with a dose calibrator,1/8" lead receptacle,12" forceps and syringe shields. A 1/8" thick lead sheet will be placed letween drawing stations, All radiopharmaceuticals will be dispensed in this area, which is located in the center of the facility in the restricted area. A refrigerator will be utilimi to store both radiopharmaceuticals, utilizing appropriate shiciding, and non-radioactive kits needing refrigeration.

'Ihe material which we work is for use by physicians for medical purposes and tlerefore is comprised of short-lived radiopharmaceuticals.

Very little danger wouki exist in case of a fire or disaster, Imwever, precaution should be exercised by fire fighting personnel should it be necessary to enter the room in which the radioactive materialis stored. In the case of a fire, the nomslatile material woukt remain confined to this room due to the nature of this building's construction. Please see building layout,9.1,9.2, and 9.3, Should it become necessary to enter the pharmacy area. survey instruments are readily available, located in rooms adjacent to the radioactive storage room. Also, personnel trained in the use of survey instruments and familiar with hazardous radiation levels would be available to assist your Jersonnel.
