G. and Alice Bergee prior to my establishing KQC. The materials license, the gauges, and the other lab equipment will be assigned to KQC as soon as I receive my State of Alaska business license and my NRC Haterials Licens Layout of KOC Lab at 306 High Street. Kodiak, Alaska, please 2) ' refer to the attached figures 1, 2, and Figure 1 is an as-built survey of 306 High Stree The lab and gage storage is on the ground floor in the two story detached garage structure fronting the roa Figure 2 is a floor plan sketch of the two floor The bottom floor has a locking garage door and a locking 30" doo Access to the upper floor is from a 30" door on the rear of ' the structur The upper story houses an office and a residential apartmen Figure 3 is a Radiation profile for the Troxler 3411B gaug . This shows that the total radiation present at the top of the transport case (i.e. handle end), with gauge enclosed, is 0.30 mREH/h As radiation intensity declines as the inverse square of distance from the source, the radiation level is only 0.02 mREH/hr 1 meter above the transport cas With two gauges stored, the radiation level would approach 0.04 mREH/hr at a height of 1 meter above the transport case Since the floor of the second story apartment / office is around 1.5 -2 meters above the top of the transport case, radiation levels in the second floor will always be below 0.04 mREH/hr. This is well below the maximum level of approximately 0.6 mREH/hr allowed by part 20.105.(b)(2) for unrestricted area l -_ - - - -

When using the gauge, follow operating procedures in accordance with the Troxler instruction manual, the NRC regulations, and this safety progra .
