INDIAN POINT 2 REACTOR OPERATOR AND SENIOR REACTOR OPERATOR INITIAL EXAMINATION RETAKE REPORT NO.

05000247/2005301

Dear Mr. Dacimo:

This report transmits the results of the Reactor Operator (RO) and Senior Reactor Operator (SRO) licensing retake written examination conducted by the NRC on April 29, 2005. This examination addressed areas important to public health and safety and was developed and administered using the guidelines of the Examination Standards for Power Reactors (NUREG-1021, Revision 9).

Based on the results of the examination, one Reactor Operator applicant passed the examination. One Senior Reactor Operator applicant failed the examination. On June 7, 2005, final examination results, including individual license numbers, were given during a telephone call between Mr. Stephen Joubert and Mr. Joseph DAntonio.

In accordance with 10 CFR 2.390 of the NRC's "Rules of Practice," a copy of this letter and its enclosure will be available electronically for public inspection in the NRC Public Document Room or from the Publicly Available Records (PARS) component of NRCs document system (ADAMS). These records include the final examination and are available in ADAMS (Master File - Accession Number ML050700339; RO and SRO Written - Accession Number ML051640286. SUMMARY OF FINDINGS

IR 05000247/20005301; April 29, 2005; Indian Point 2; Initial Operator Licensing Examination.

One Reactor Operator out of two applicants passed the examination (one reactor operator and one SRO instant).

The written examinations were administered by the facility. The operating tests were waived by Region 1 since the applicants had passed that portion of their original examinations. There were no inspection findings of significance associated with the examinations.

REACTOR SAFETY

Mitigating Systems - Reactor Operator (RO) and Senior Reactor Operator (SRO) Initial License Examination

a. Scope

of Review The NRC examination team developed the written and operating initial examination and together with Indian Point 2 training and operations personnel verified or ensured, as applicable, the following:

  • The examination was prepared and developed in accordance with the guidelines of Revision 9 of NUREG-1021, "Operator Licensing Examination Standards for Power Reactors. Final resolution of comments and incorporation of test revisions were conducted prior to the administration of the examination.
  • A test item analysis was completed on the written examination for feedback into the systems approach to training program.
  • Examination security requirements were met.

The written examination was administered by the Indian Point 2 training staff on April 29, 2005.

b. Findings

Grading and Results One of two applicants (one SRO and one RO) passed all portions of the initial licensing examination.

The facility had no post-examination comments.

Examination Administration and Performance No findings of significance were identified.

4OA5 Other (Inadvertent Potential Initial License Exam Security Compromise)

The facility had assigned the examination author to develop three periodic quizzes for the initial license class in addition to the comprehensive audit and actual NRC written examinations. During the development of the written examination, the facility determined by review of industry operating experience that their use of the NRC examination author for exam development other than the NRC and audit examinations was not in compliance with the requirements of NUREG-1021 Operator Licensing Examiner Standards. A review was performed of these examinations by the facility and the NRC and the proposed written examination to determine if a potential exam compromise existed. This review found two duplicate and four similar questions on the proposed written examination vs. the quizzes. These questions were replaced.

The use of the NRC exam developer to develop license class examination materials is a violation of 10 CFR 55.49 because, but if for detection, it would have affected the equitable and consistent administration of the examination. This finding was considered minor in that it did not have any actual safety consequences, the issue was not a precursor to a significant event, if left uncorrected it was not likely to become a more significant event, the issue did not relate to a performance indicator, and the issue was not directly associated with one of the cornerstones objectives. Although this issue constitutes a violation of minor significance that is not subject to enforcement action in accordance with Section IV of the Enforcement Policy, it is being documented because it is associated with an issue of agency wide concern (i.e., NRC exam security, NUREG-1021, Draft Revision 9, ES-501, E.3). The facility placed this issue in its corrective action program (condition report number IP2-2005-1385).

4OA6 Exit Meeting Summary

On June 7, 2005, the NRC provided conclusions and examination results to Indian Point 2 management representatives via telephone. A license number for the one applicant who passed the examination was also provided during this time.

The NRC expressed appreciation for the cooperation and assistance that was provided during the preparation and administration of the examination by the licensees training staff.
