This is in reference to your two letters dated May 31, 1980, in response to the Notice of Violation and Notice of Proposed Imposition of Civil Penalties sent to Providence Hospital by letter dated May 7, 1980.
The letter dated May 7,1980, concerned items of noncompliance found during a special inspection conducted by the Office of Inspection and Enforcement on January 28 and 29, 1980, of activities authorized by NRC License No. 50-17838-01'at Providence Hospital in Anchorage, Alaska.
The special inspection related to loss of radioactive materials by Providence Hospital.
After careful consideration of your responses we have concluded that the items of noncompliance did occur. and that civil penalties should be imposed as set forth in the enclosed Order.
After consideration of your response to Item 3 of the. Notice of Violation, we have remitted that item of noncompliance, and no penaltiies are imposed for Item 3.
Accordingly, we hereby serve the enclosed ORDER on Providence Hospital, imposing civil penalties in the cumulative amount of One Thousand Five Hundred Dollars ($1,500).
The implementation of corrective actions described in your letters will be examined during our future inspections, and the results of our inspectione will be considered in determining whether any further enforcement action, such as modification, suspension or revocation of your license, is appropriate.
