Gentlemen:

This is in reference to your letter dated July 28, 1986 to amend your byproduct

material license. In order to complete our review, we need the following

additional information:

Your letter states that the bone mineral analyr.er will be located in the

Providence Professional Building. You should submit the street address

for that facility, as well as the appropriate room number (s). Also,

submit a letter from the hospital administrator confirming that

Providence Hospital is responsible for the use of the bone mineral

analyzer in the Providence Professional Building.

2.

10 CFR 20.207 requires licensed material to be restricted from

unauthorized removal. You should describe how keys will be controlled

so that only authorized users will have access to the bone mineral

,

analyzer.

We will continue the review of your amendment request upon receipt of this

information. If we do not teceive a reply from you within 30 calendar days

from the date of this letter, we shall assume that you do not wish to pursue

your application. Please reply in duplicate, and refer to Mail Control No.
