ENCLOSURE

NOTICE OF VIOLATION Pacific Gas and Electric Company Docket:

50-323 Diablo Canyon Nuclear Power Plant License:

DPR-82 During an NRC inspection conducted on January 8 through February 18, 1995, one violation of NRC requirements was identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"

CFR.Part 2,

Appendix C, the violation is listed below:

A.

Diablo Canyon -Technical Specification 6.8. I states, in part, that written proced,.;es shall be established, implemented, and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, dated February 1978.

Appendix A of Regulatory Guide 1.33, Revision 2, recommends procedures involving the operation of the control rod drive system.

iievision 12 of Procedure OP A-3: IV, "Control Rod System - Manual Operation of Reactor Trip and Bypass Breakers,"

Step 5.2, states.

in part, that instruction steps (denoted by sign-off spaces)

in this procedure will be performed using concurrent verification

. and, as such, require two operators; one to manipulate the components, and the other to verify the accuracy of the manipulation prior to it taking place.

'Revision 12 of OP A-3: IV, Step 6.4.4, states,

" Install the breaker racking bar and lift up on the bar to release tension on the spring loaded locking device."

Revision 12 of OP A-3: IV, Step 6.6.3, states, racking bar and lift up on the bar to release spring loaded locking device."

)995, the racking bar was not installed during performance of either of the above Steps 6.4.4 or 6.6.3, yet the operators signed that both performance and verification were accomplished For each step.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Pacific Gas and Electric Company is hereby required to submit a written statement or explanation to the U.S.

Nuclear Regulatory Commission, ATTN:

Document Control Desk, Mashington, D.C.

20555 with a copy to the Regional Administrator, Region IV, 611 Ryan Plaza Drive, Suite 400, Arlington, Texas 7601 1, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date, of the letter transmitting this Notice of Violation (Notice).

This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation:

(I) the reason for the violation, or., if contested.
