NRC INSPECTION REPORT 150-00042/99-09 (FORM-591)
Dear Mr. Hicks:
This letter and the enclosed NRC Form-591 refer to the safety inspection conducted on August 18,1999, at your facility in Mont Belvieu, Texas. The inspection included a review of activities authorized by 10 CFR Part 150. The preliminary inspection findings were discussed with you at the conclusion of the inspection. The final inspection findings were presented to you by telephone on August 23,1999.
Based on the results of this inspection, the NRC has determined that two violations of NRC requirements had occurred. The violations are cited on the enclosed NRC Form-591. Pursuant to the provisions of 10 CFR 2.201, you are required, within 30 days of the above date, to sign and date the NRC Form-591, indicating that appropriate corrective actions have been or will be taken to correct the violations described on the form. After printing your name, signing and dating the NRC Form-591, please remove the top copy for your files and return the remaining copies to me at the address above.
