Dear Sir or Madam:

Attached is the Owner's Activity Report, OAR-1-00008 as required by Section XI of the ASME Code (Code). The report was prepared and submitted in accordance with the requirements of ASME Code Case N-532-1 as authorized by the NRC.

The report contains an overall summary of Insetvice Inspections performed during the Third Period of the Second Interval. The Third Period of the Second Interval started June 2004 and will end June 2007. The report has been certified in accordance with Code requirements.

Note 1- Two category B-A exarkations were completed with known limitations, relief request will be generated to obtain the limited examination approval from the NRC.

Note 2 - Thirteen category B-D examinations were completed with known limitations, relief request will be generaled to obtain the limited examination approval from the NRC.

Note 3 - Two category B-F welds were deferred to the next refueling outage due to weld configuration issues, relief request will be generated to extend the second ten year interval in order to complete the required examination population.

Note 4 - The total required numbers do not represent the B-G- 1 bolting which requires examination only when disassembled.

One category B-G-1 examination was completed with known limitations, relief request will be generated to obtain the limited examination approval from the NRC.

Note 5 - The total required numbers do not represent the B-G-2 bolting which requires examination only when disassembled.

Note 6 - Seventy one category B-J welds were deferred until the next outage in accordance with approved relief request GG-ISI-003 and one category B-J weld was deferred due to the weld configuration problems.

Note 7 - Category B-H, and B-K-1 items being examined under the rules of N509. Category B-H, exam was a limited examination, relief request will be generated to obtain the limited examination approval from the NRC.

Note 8- Examination only required if pump or valve is disassembled for maintenance, therefore, percentages are not applicable.

Note 9 - Category B-0 components are not accessible for examination, relief request will be generated to obtain approval from the NRC for not performing the examination.

Note 10 -All three category C-F-1 welds were added to the IS1 program during the second period and the one weld selected for the third period (RF15) was deferred until the next outage in accordance with approved relief request GG-ISI-003.

Note 11 - Four category C-F-2 welds were deferred until the next outage in accordance with approved relief request GG-ISI-003.

Note 12 - Two category C-G welds were not accessible for examination during the third period, relief request will be generated to obtain the limited examination approval from the NRC.

Note 13 - Five (5) of the twelve (12) Class 1 test include high pressure water LLRT performed in conjunction with the 10 year hydro Note 14- Twenty-four (24) Class 2 test were deleted by code case N522 and are reflected by reducing period 3 Class 2 test by Twenty-four (24).
