On February 17, 2026, the licensee notified the Department that the shutter on a Vega SH-F2 gauge, which contains a 500-millicurie (original activity) cesium-137 source, failed to close during testing. The licensee reported that a contractor will be contacted to repair the gauge shutter. The licensee also stated that there is no risk of additional radiation exposure to members of the general public or radiation workers due to this on/off mechanism failure. Additional information will be provided as it is received, in accordance with SA-300.
