Title	Agreement State Report - Process Gauge Stuck Shutter
Event Description	The following report was received via e-mail: On November 10, 2017, the Agency was notified by the licensee's service provider that during routine test of an Ohmart Vega model SHD-45 nuclear gauge, the shutter failed to close. Open is the normal operation position. The gauge contains a 250 millicurie (original activity) cesium-137 source. The service provider stated the gauge is not an exposure risk to any individual. The service provider stated no corrective action has been determined. Texas Incident: I-9520
