| Event Description | The following information was received via E-mail:
On July 19, 2021, the licensee notified the Agency [Texas Department of State Health Services] that one of its radiographer's personal dosimetry badge results for the monitoring period of June 2021 indicated a deep dose equivalent (DDE) of 5,114 millirem. The radiographer had terminated his employment with the licensee on July 6, 2021. The licensee has contacted the radiographer by phone and the radiographer stated he did not know how it could have happened. The licensee is investigating to determine if the dose was to the radiographer or to the badge only. The licensee also reported that the radiographer had more than one day of work and that the dose to this badge did not occur all within a 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> period.
