| Title | Agreement State Report - Source Shipment Did Not Arrive as Expected |
| Event Description | QSA Global reported a lost source. The location of the source is not known. The shipment contains Ir-192.
QSA expected this source to arrive at their Baton Rouge, LA location at about 0800 CDT on 5/6/13. QSA contacted the common carrier to locate the item at some time after that. The last known location was Memphis, TN.
At about 1915 EDT, QSA personnel learned from the common carrier that the item had been located at the Memphis, TN shipping location. THIS MATERIAL EVENT CONTAINS A "CATEGORY 2" LEVEL OF RADIOACTIVE MATERIAL Category 2 sources, if not safely managed or securely protected, could cause permanent injury to a person who handled them, or were otherwise in contact with them, for a short time (minutes to hours). It could possibly be fatal to be close to this amount of unshielded radioactive material for a period of hours to days. These sources are typically used in practices such as industrial gamma radiography, high dose rate brachytherapy and medium dose rate brachytherapy.
