On January 3, 1986, Unit 2 was in Mode 6 " REFUELING" when it was discovered that 42ST-2SE01 (Source Range Flux Monitor Channel Checks) was not performed within the Technical Specification maximum allowable time.

To prevent recurrence, the following actions have been taken by the Shift Supervisor:

1.

Instructed the Reactor Operators (RO) to review Surveillance Tests (ST) on shift turnover.

2.

Instructed the RO's that they should, if there are any questions on operability or time to perform ST's, contact the Assistant Shift Supervisor or the Shift Supervisor.

3.

Reviewed the ST with the RO's to ensure that they know how to evaluate when performance is required.

4.

Emphasized that the shift ST's are within the RO's area of responsibility for initiation and performance.

5.

Instructed Assistant Shif t Supervisor to review the ST's to know when they are due.

A step will also be added to 42ST-2ZZ19 (Routine Surveillonce Mode 5-6 Logs) to initiate performance of 42ST-2SE01 in Mode 6.

Technical. Specification 4.9.2 Surveillance Requirement is to perform a CHANNEL CHECK at least once per 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

The surveillance was performed on January 2, 1986, at 0820 and then again on January 3, 1986, at 0030, a total time span of 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br /> and 10- minutes.

The Technical Specification requirement is every 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> with a maximum allowable extension not to exceed 25% of the surveillance interval which is an additional 3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br />.

The maximum allowable interval for this surveillance is 15 hours1.736111e-4 days <br />0.00417 hours <br />2.480159e-5 weeks <br />5.7075e-6 months <br />, which was exceeded by 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> and 10 minutes.

42ST-2SE01 was performed again within 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> to return to the normal performance schedule.

The root cause of the incident was the Licensed Operator's failure to perform. the test at the scheduled time.

To prevent recurrence, the following actions have been taken by the Shift Supervisor:

1.

Instructed the Reactor Operators (RO) to review Surveillance Tests (ST) on shift turnover.

Instructed the RO's that they should, if there are any questions on operability or time to perform ST's, contact the Assistant Shift Supervisor or the Shift Supervisor.

A step will also be added to 42ST-22219 (Routine Surveillance Mode 5-6 Logs) to initiate performance of 42ST-2SE01 in Mode 6.

The source ranges do not provide any automatic safety feature actuations and were within the acceptance criteria as stated in 42ST-2SE01 when the surveillance was performed, therefore, there was no adverse impact on the safety of the plant.

No similar events have occurred previously in Unit 2.
