Description

10 CFR Part 50, Appendix B, Criterion III, Design Control, required, in part, that design control measures shall provide for verifying or checking the adequacy of design, such as by the performance of design reviews, by the use of alternate or simplified calculation methods, or by the performance of a suitable testing program. Contrary to this, on December 4, 1987, the licensee failed to ensure design measures were in place for verifying or checking the adequacy of AFW hatch cover plate design. Specifically, in Calculation 5.6.3.9, the licensee failed to ensure that a safety factor in accordance with the station design criteria was applied in the design of expansion anchors. The issue was identified in the licensees corrective action as IR 654270. This finding was of very low safety significance because it did not represent an actual open pathway in the physical integrity of reactor containment
