| Title | Licensee-Identified Violation |
| Description | Technical Specification (TS) 3.3.6.1, Primary Containment and Drywell Instrumentation, requires the primary containment and drywell isolation instrumentation be operable while in Modes 1, 2, and 3. Contrary to the above, on August 3, 2013, the licensee failed to ensure the primary containment and drywell isolation instrumentation was operable prior to changing from Mode 4 (Cold Shutdown) to Mode 2 (Startup). On August 6, 2013, during a supervisory review of procedures in progress, the licensee determined that they were not incompliance with TS 3.3.6.1 due to jumpers that were installed to disable the function of the instrumentation. The licensee immediately entered the TS 3.3.6.1 Limiting Condition for Operation and associated actions. The licensee restored compliance with the TS by removing the jumpers and restoring the primary containment and drywell instrumentation to operable status and documented this issue in the corrective action program under Condition Report CR-GGN-2013-5101. Using Manual Chapter 0609, Appendix A, Significance Determination Process for Findings at Power, dated June 19, 2012, the inspectors determined that this finding had very low safety significance (Green) because it did not represent an actual open pathway in the physical integrity of the reactor containment or drywell and did not involve the hydrogen igniters in the reactor containment. |
