Procedure,
EN-QV-111, Training and Certification of Inspection/Verification and Examination Personnel, Section 4.0 [4](i), requires that the Entergy corporate ANSI Level III inspector shall perform periodic (annual) surveillances of quality control inspection activities to ensure that the program is being adequately implemented and maintained. Contrary to the above, no surveillances of quality control inspection activities were performed for any Entergy site during calendar year 2008. The issue was not suitable for quantitative significance determination, so it was assessed using
IMC 0609, Appendix M, so it was evaluated using the qualitative criteria listed in Table 4.1. This finding was determined to be of very low safety significance because other quality assurance program functions remained unaffected by this performance deficiency, so defense-in-depth continued to exist. This issue was entered into the licensee\'s corrective action program as
CR-HQN-2009-00111.
