E. CORRECTIVE ACTIONS

The emergent Freon leak on the 'A' train of VC/VE was repaired and the train was restored to operable status at 0908 hours0.0105 days <br />0.252 hours <br />0.0015 weeks <br />3.45494e-4 months <br /> on November 23, 2013.

The failed motor was replaced and the 'B' train of VC/VE was returned to service on November 27, 2013.

The failed motor was sent offsite for failure analysis.

An Equipment Apparent Cause Evaluation will be performed to determine the cause of the winding failure identified by the failure analysis.

PREVIOUS OCCURRENCES

A document search identified no occurrences in the previous 10 years of a reportable condition caused by the failure of a ventilation fan motor. The 'B' VE Cooler Condenser Fan motor had been in-service since February 2, 1987.
