At 2148 CST 2/14/79 while perfonning startup test procedure HNP-2-10925, MSIV clos-l ure, safety relief valves 2B21-F013 C and G failed to open with its 1090 Psig set-I 1 o 13 l l o 4 l

points in the presence of a reactor high pressure of 1125 Psig.

Safety relief valvej C and G were declared inoperable and a maintenance request was initiated to replace ;

lo Isl 1 C and G with new topworks of the same pressure setpoints.

The other 9 safety reliefg lo lc l 1

valves were available at the time of the scram.

This is a non-repetitive occurrence l o

7 for Unit 2, but this event has repetitively occurred on Unit 1.
