10 CFR 50, Appendix B, Criterion III requires in part that design changes, including field changes, shall be subject to design control measures commensurate with those applied to the original design and be approved by the organization performing the original design, unless the applicant designates another responsible organization. Contrary to the above, the licensee made modifications to the design operating pressure of the Unit 1
CCW System Valves HV3096A and HV3096B which isolate
CCW flow to and from the non-safety related evaporator packages and
hydrogen recombiners. These modifications were made using Calculation SM-99-2189-017, which had been revised with errors in regulator settings for the above valves. The licensee identified the errors in the calculation following changes to regulator settings and entered the error into their
CAP. Station personnel performed an operability evaluation of these valves and determined compensatory measures were necessary until their regulator settings could be readjusted to their correct design setpoints
