Failure to Identify a Condition Adverse to Quality on Division 2 Residual Heat Removal Service Water Outlet Flow Control Valve

Description

A finding of very low safety significance and associated non-cited violation of 10 CFR Part 50, Appendix B, Criterion XVI, Corrective Action, and TS 3.7.1 Residual Heat Removal Service Water (RHRSW) System, were self-revealed for the licensees failure to identify a condition adverse to quality on the Division 2 RHRSW outlet flow control valve E1150F068B. Specifically, troubleshooting and the associated post maintenance testing failed to identify and correct a failed anti-rotation key which resulted in an inoperable Division 2 RHRSW system for longer than its TS 3.7.1 allowed outage time.
