Title

Failure to Ensure Electrolytic Capacitors Installed in the Plant Did Not Have Expired Shelf Lives

Description

A finding of very low safety significance with an associated non-cited violation of 10 CFR 50, Appendix B, Criterion VIII, Identification and Control of Materials, Parts, and Components was self-revealed when the reactor water cleanup system inlet flow square root converter failed, resulting in a failure of the reactor water cleanup (RWCU) differential flow instrument and loss of automatic isolation function of the RWCU isolation valves. Specifically, electrolytic capacitors were installed in the RWCU system logic that had expired shelf lives, resulting in failures of the automatic isolation function of the RWCU system.
