| Description | As described in Section 4OA3.2 of this report, the licensee discovered control center pressure boundary door R5-6 inappropriately blocked open on March 26, 2008. The licensee determined that Procedure 35.000.242 did not identify the door as a control center pressure boundary as it should have. This procedure deficiency allowed operations personnel to block the door open without adequate compensatory measures. The licensee entered this issue into their corrective action program as CARD 08 22059, removed the drain hose, closed the door, and revised the procedure. 10 CFR 50, Appendix B, Criterion V, Instructions, Procedures, and Drawings, requires, in part, that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances. Contrary to the above, on March 24, 2008, the licensee utilized an inadequate procedure when the control center pressure boundary was breached. Specifically, Procedure 35.000.242 failed to identify auxiliary building door R5-6 as a control center pressure boundary door. Because this issue also affected the toxic gas and smoke barrier function, the SRA performed a phase 3 analysis and determined that the issue was of very low safety significance (Green) |
