| Description | Licensee TS 5.7.1 requires that each HRA area as defined in 10 CFR 20 be barricaded and conspicuously posted as an HRA. Contrary to the above, on January 16, a station operator positioned and left in place a ladder into the overhead of the torus basement in Bay 16 to access valve G3300F124, which made a previously inaccessible HRA of the torus basement overhead accessible to plant personnel and resulted in an unposted ad unbarricaded HRA. Immediate corrective actions included posting and controlling the overhead as an HRA, and performance management of the individuals involved. The violation is of very low safety significance because the problem did not cause an overexposure, a substantial potential for an overexposure, or compromise the licensees ability to assess dose. The licensee entered this issue into their corrective action program as CARD 08-20282 |
