On September 11, 2013, while operating at 100% power, Boron-10 Areal Density Gage for Evaluating Racks (BADGER) test results from June 2013 testing were received by NextEra Energy Duane Arnold, LLC. The results indicated that the areal density used in the criticality analysis of record may not be supported. Specifically, the report indicated that the areal density assumed in the criticality analysis of record as discussed in UFSAR Section 9.1.2.2.1 may not be supported. This conclusion was based on eleven of the tested panels having an average areal density (mass per unit area) below the minimum of 0.0232 gm-B 10/cm 2 (grams of Boron-10 per squared-centimeter) and nineteen of the tested panels having a minimum (2-inch) areal density below the minimum of 0.0232 gm-B 10/cm2. Since the previously noted assumed minimum areal density in the criticality analysis cannot be supported based on testing results, the PaR spent fuel pool racks do not conform to K-infinity and enrichment limits of DAEC Technical Specification (TS)
Design Specifications 4.3.1.1 (i) and 4.3.1. 1 (iii). Therefore, the racks were determined to be Operable but Non-Conforming. This event resulted in a Condition Prohibited by TS and is reportable pursuant of 10 CFR 50.73(a)(2)(i)(B). A Root Cause Evaluation determined the root cause to be insufficient margin to allow for the uncertainty added by BADGER testing.

IV.
Corrective Actions
As an interim corrective action, STP 4.3.1-01, "Fuel Storage Facilities K-Infinity Limit Checks" was revised to provide more rigorous controls on the PaR rack cells to ensure fuel with K-infinity > 1.29 is not loaded or stored in the PaR racks.
Corrective actions to prevent recurrence will be taken to revise the criticality analysis to include additional margin for B 10 areal density to bound testing uncertainty to support future neutron attenuation testing.
This action will also include revising the UFSAR and TS in order to address the Operable but Non-Conforming condition.
