Description

Aself-revealed Green non-cited violation(NCV)of Title 10 Code of Federal Regulations(10 CFR) 20.1501, Surveys and Monitoring: General, was identified when Exelon failed to perform adequate surveys of the 11 reactor coolant pump bay area following the aggregation of 25 high dose-rate in-core detectors in one area of the flooded refueling cavity, which is adjacent to the pump bay. Surveys were not performed as required after radiological conditions changed and radiological hazard mitigation measures, such as locking and controlling access in accordance with Exelon procedures, were not implemented, resulting in accessible dose-rates of up to 2,000 millirem per hour(mrem/hr)in the pump bay
