DPR-54 2/ REPORTABLE OCCURRENCE 81 ^9 / /3 / kll / In accordance wi th Rancho Seco Nuclear Generat ing Stat ion Technical Specifications Section 6.9.4.2.b, and Regulatory Guide 1.16, Section C.2.b(2), the Sacramento Municipal Ut ili ty Dist rict hereby submits a thirty-day report of Reportable Occurrence No. 81-49 On September 29, 1981, with the reactor in cold shutdown, safety features valve SFV-22025 f ailed to close during performance of surveillance test SP 203.03 The valve was inspected by maintenance personnel and the Limitorque operator, model SMB-00, spring pack was found to be loose. Specifically, a gap was found between the retaining nut and the washer. Normally, this spring pack is pre-loaded, thus there should bt no gap. The result of this looseness was that the torque limit s sitch was actuated prematurely which in turn caused the valve to fm.1 to stroke. This problem has nc t been observed on other Limitorque operators and is apparently caused by improper set-up on this unit at the factory. The spring pack was removed and returned to Limi.7rque. A new spring pack was put back i n the operator and the valve was tested for proper s t roke. A typical drawing of the Limi torque operator showing the affected parts in enclosed.
