| Description | Technical Specification Limiting Condition for Operation 3.10.4 requires, in part, that to allow withdrawal of a single control rod with the reactor in Mode 4, all other control rods in a five by five array centered on the control rod being withdrawn are disarmed. Condition B.2.2 requires when a limiting condition for operation is not met with the affected control rod not insertable to immediately initiate actions to satisfy the requirements of this limiting condition for operation. Contrary to the above, on November 1, 2009, the licensee discovered that the control rods in the five by five array around a withdrawn control rod were not disarmed for over two hours without immediately taking the actions required by technical specification action statement B.2.2. This was documented in the licensees corrective action program by CR-CNS-2009-9138. Because the plant was shutdown at the time this performance deficiency occurred, the inspectors used Manual Chapter 0609, Appendix G, Shutdown Operations Significance Determination Process. Using Checklist 7 in Attachment 1, Shutdown Operations Significance Determination Process Phase 1 Operational Checklists For Both PWRs and BWRs, the inspectors determined that the finding had very low safety significance because it did not require quantitative assessment for a phase 2 or 3 analysis |
