Title | Failure to Perform Leak Rate Testing on Containment Boundary Piping |
Description | The inspectors identified a Green non-cited violation of 10 CFR 50, Appendix J, Primary Reactor Containment Leakage Testing for Water-Cooled Power Reactors, for Exelons failure to establish an adequate program that leak tested components penetrating the primary containment pressure boundary. Specifically, Exelon failed to implement leak rate testing of the reactor building (RB) normal closed loop cooling piping to verify piping integrity to support its containment isolation function. As a result, on November 10, 2013, engineering personnel identified an inoperable containment isolation boundary due to a degraded RB closed cooling piping condition. Exelon documented this issue in issue report (IR) 1598590 and took corrective actions to revise the Appendix J test program and address the missed leak rate surveillance test. This finding is more than minor because it is associated with the Barrier Performance attribute of the Barrier Integrity cornerstone and affected the cornerstone objective to provide reasonable assurance that physical barriers, as designed, protect the public from radionuclide releases caused by accidents or events. Specifically, Exelon failed to perform leak rate testing of the RB normal closed loop cooling piping and failed to identify the degraded piping condition that impacted the containment isolation function. In accordance with IMC 0609.04, Initial Characterization of Findings, and Exhibit 3 of IMC 0609, Appendix A, The Significance Determination Process for Findings At-Power, issued June 19, 2012, the inspectors determined that the finding did not represent an actual open pathway in the physical integrity of the reactor containment isolation system nor did it involve an actual reduction in function of hydrogen recombiners for the reactor containment therefore, the finding was of very low safety significance (Green). The finding was not assigned a crosscutting aspect because the most significant causal factor of the finding was the failure to implement leak rate testing since 1991 and was not indicative of current plant performance. |
