The shift chemist did not take a grab sample as required by Technical Specification Table 3.8.1 when the Service Water Discharge Pipe Gross Radioactivity Monitor was removed from service.
Two Preventative Maintenance Procedures were scheduled for the day shift, February 6,1995: the ennual Service Water Radiation Monitor Pump replacement and the quarterly Service Water Radiation Monitor strainer cleaning. Operations personnel removed the monitor from service at 0530 in preparation for the maintenance work. A grab sample was then required by 1330. This sample was not taken until 1503. Administrative changes have been made to assure that samples that are required on Technical Specification equipment removed from service are taken as required.

Description
While operating at 100% power, maintenance was planned to be performed on the Service Water Discharge Pipe Gross Radioactivity Monitor (Ells System Code: IL)(Ells Component Code: RI). Two Preventative Maintenance (PMs) procedures were '
scheduled to be performed on the day shift, February 6,1995: the annual Service Water Radiation Monitor Pump (Ells Component Code: P) replacement (PM 4123) and the quarterly Service Water Radiation Monitor Strainer (Ells Component Code: STR) cleaning (PM 4235). Operations personnel removed the monitor from service at 0503 on February 6,1995 in preparation for the maintenance work.

Operations personnel notified the shift chemist at 0530 that the monitor was out of service and grab samples were required every 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />. This information was passed to the day shift chemist at the shift turnover. The control room operators and the day shift chemist discussed the grab sample around 0800 when the shift chemist made their rounds through the control room. The shift chemist indicated that the sample would be taken later if the monitor was not back in service within the 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.
At 1500 the control room operator contacted the chemist to verify the sample had been taken and learned that it had not been taken.
Cause
There were no administrative aids to ensure that the shift chemist took the required samples.
Analysis This event is reportable per 10 CFR Part 50, Section 50.73(a)(2)(i) since a Technical Specification required test was not performed within the specified time.

The Service Water Discharge Pipe Gross Radioactivity Monitor measures the radioactivi s of the service water after it has passed through various heat exchangers I
and before it is mixed with the Circulating Water (See Figure). This mixed water is sent to the Discharge Canal (after being sent to the Cooling Towers if they had been in service). The Discharge Canal water is continuously monitored by two other radiation monitors (the Discharge Canal Gross Radioactivity Monitors), which were in service.
These monitors use one of two sample pumps to continuously send water through a radiation monitor and also periodically discharges a sample to a collection tank that is analyzed weekly (called the composite sample). Both the continuous monitors and the composite sample confirmed that no radiological releases occurred during the 10 hours1.157407e-4 days <br />0.00278 hours <br />1.653439e-5 weeks <br />3.805e-6 months <br /> that the monitor was out of service.
There were no consequences from missing this Service Water grab sample since no radiological releases were made and the water was continuously monitored by the Discharge Canal Gross Radioactivity Monitors.
The sample was taken upon discovery of the error, indicating only naturally occurring radioactivity.
Corrective Actions
1.
The sample was immediately taken following discovery of the error.
2.
The shift chemist's office white board has been modified to provide a designated area for sample times for Technical Specification equipment which is not in service. Procedures have been modified to require the shift chemist to write on the white board the time that future samples will be required to be taken.

However, the reasons for missing the sample in 1987 were associated with a misunderstanding between the Operations Group and the personnel doing the sampling. Therefore, the corrective actions could not have prevented this event.
