REPORTABLE OCCURRENCE 251-81-13 Licensee Event Report Page two Additional Cause Description and Corrective Actions The violation was discovered immediately and corrected within about two (2) minutes. The interlock was reinstated within the following 30 minutes.

l Investigation revealed that the interlock mechanism was operable approx!mately one (1) hour prior to the above observation.

The item that was found to be at fault is commonly referred to as a " Dog Ear" in the air lock side of the inner door. It appeared that the " Dog Ear" had been knocked out of p0$ition thus allowing the door latch mechanism to be placed in the

" Latched" position with the door remaining open. The mechanical interlock set in the " Latch" position allowed the outside door to open thus leaving both doors open.

The " Dog Ear" was repositioned and the operation of the doors tested and verified to work properly.

l The following additional corrective action will be taken:

All plant and contract personnel will be instructed on the importance of maintaining containment integrity as required by our Technical Specifications.

2)

A Plant Change and Modification (PC/M) is being written to provide the following:

a)

Limit switches which sense actual door position, rather than latching mechanism position, will be added.

b)

Additional circuitry will be installed to provide a redundant actuation of the " Interlock Violated" alarm.
