ATTACHMENT TO LER 81-08/3L NORTHEAST flVCLEAR EllERGY COMPANY MILLSTONE NUCLEAR POWER STATION - UNIT 1 PROVISIONAL LICEllSE flVMBER DPR-21 DOCKET flVMBER 50-245 IDENTIFICATION OF OCCURRENCE Operation in a degraded mode permitted by a limiting condition for operation occurred when the emergency diesel generator failed to start.

CONDITIONS PRIOR TO OCCURREllCE Prior to the occurrence, the unit was in cold shutdown for performance of main turbine repairs.

pESCRIPTION OF OCCURRENCE On April 27,1981, at.1230 hours0.0142 days <br />0.342 hours <br />0.00203 weeks <br />4.68015e-4 months <br />, while performing routine surveillance, Diesel Generator Operational Readiness Demonstration, the engine tripped on high crankcase pressure. Technical Specification 4.9.1.a requires that the diesel generator be started and loaded once a month to demonstrate operational readiness.

DESIGflATION OF APPARENT CAUSE OF OCCURRENCE The Diesel Generator engine tripped on high crankcase pressure due to defective hoses on the crankcase ejector, which allowed air leakage.

ANALYSIS OF OCCURRENCE The Diesel Generator is provided to insure adequate power for operation of emergency safeguards equipment.

Failure to successfully demonstrate operational readiness of the Diastl Generator did not result in a condition that had not been previously analyzed. The Gas Turbine Generator as well as off-site power sources were available and would have provided adequate power for operation of this equipment if the need had arisen.

CORRECTIVE ACTION

The defective ejector air hose was replaced and the Diesel Generator operational readiness demonstration was successfully completed. An improved air hose supplied by the vendor has been installed to reduce the potential for this type of failure.
