All smears taken on the cask rain cover, trailer bed, and tires were less than minimum detectable activity for removable contamination. This notification is being made as a four-hour, non-emergency notification for a Notification of Other Government Agency. This event is reportable in accordance with 10 CFR 50.72(b)(2)(xi). There was no impact on the health and safety of the public or plant personnel. The NRC Resident Inspector has been notified.
ENS 5432210 October 2019 15:22:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Environmental Report to Another Government AgencyAt 1122 EDT, on October 10, 2019, Duke Energy initiated voluntary notification of North Carolina State and local officials per the guidance in Nuclear Energy Institute (NEI) 07-07, 'Industry Groundwater Protection Initiative - Final Guidance Document,' due to release of tritiated water in excess of 100 gallons. On October 8, 2019, at approximately 1300 EDT, Brunswick plant personnel drilling as part of an ongoing site project, damaged a storm drain discharge line. The resulting leak was isolated and water around the impacted area was sampled for gamma emitters and tritium. No gamma emitters were detected. The tritium concentration was below the Environmental Protection Agency (EPA) drinking water limit of 20,000 pCi/L. The leak has been stopped and excavation and repair efforts are in progress. This notification is being made solely as a four-hour, non-emergency notification for a notification of other government agency. This event is reportable in accordance with 10 CFR 50.72(b)(2)(xi). There was no impact on the health and safety of the public or plant personnel. The NRC Resident Inspector has been notified.
ENS 5341321 May 2018 04:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Regarding Malfunction of Emergency Notification SirenOn May 21, 2018, at approximately 0840 Eastern Daylight Time (EDT), Duke Energy confirmed that one emergency notification siren (i.e., B13) located in Brunswick County was malfunctioning. The siren was making an abnormal sound, alternating between low and high pitch. The siren was deactivated upon confirmation of the siren malfunction. There are a total of 38 sirens located in Brunswick and New Hanover Counties. No other sirens were affected. Duke Energy notified the State of North Carolina, Brunswick County, New Hanover County and Coast Guard of the issue. There was no impact on the health and safety of the public or plant personnel. The NRC Senior Resident Inspector has been notified.

The siren was making an abnormal sound, alternating between low and high pitch. At approximately 1103 EDT, the siren was deactivated. There are a total of 38 sirens located in Brunswick and New Hanover Counties. No other siren was affected. Duke Energy notified the State of North Carolina, Brunswick County, and New Hanover County of the issue. There was no impact on the health and safety of the public or plant personnel. The NRC Resident Inspector has been notified.
ENS 5284510 July 2017 20:02:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Contract Employee FatalityAt approximately 14:10 Eastern Daylight Time (EDT), the Control Room was notified of a contract employee experiencing a non-work related medical emergency within the protected area in the service building. First responders were immediately dispatched. Off-site assistance was requested. The individual was transported to the New Hanover Regional Medical Center. No radioactive material or contamination was involved. At 16:02 EDT, hospital officials notified plant personnel that the patient was declared deceased. This notification is being made in accordance with 10 CFR 50.72(b)(2)(xi) for a situation related to the health of on-site personnel for which a notification to other government agencies is planned. The Occupational Safety and Health Administration (OSHA) will be notified. The NRC Resident Inspector has been notified.
ENS 5208011 July 2016 21:45:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Non-Work Related On-Site Contractor FatalityAt approximately 1614 Eastern Daylight Time (EDT), the Main Control Room was notified of a contracted employee experiencing a non-work related medical issue near the Discharge Weir. First Responders were immediately dispatched and present at the scene in minutes. Off-site assistance was requested and arrived on-site at approximately 1641. Dosher Memorial Hospital officials notified the plant at approximately 1745 that the patient was declared deceased at 1710. The individual was outside of the protected area (within the owner controlled area), and no radioactive material or contamination was involved. The cause of death has not yet been determined. This notification is being made in accordance with 10 CFR 50.72(b)(2)(xi) for situation related to the health of on-site personnel for which a notification to other government agencies is planned. The Occupational Safety and Health Administration (OSHA) will be notified. The NRC Resident Inspector has been notified. This event did not result in any adverse impact to the health and safety of the public. The licensee completed their notification to OSHA at 1835 hours.
ENS 518485 April 2016 13:50:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Employee FatalityThis is a non-emergency four hour notification for a Notification of Other Government Agency. This event is reportable in accordance with 10 CFR 50.72(b)(2)(xi). At approximately 0908 Eastern Daylight Time (EDT) on April 5, 2016, an employee developed a personal medical condition in the Protected Area at the Brunswick Nuclear Plant. The Brunswick County Sherriff and local Emergency Medical Services were notified and they responded and attempted to resuscitate the unresponsive employee. These efforts were unsuccessful and the individual was declared deceased at approximately 0950 EDT on April 5, 2016. The fatality was not work related and the individual was inside of the Radiological Controlled Area and was not contaminated. No news release by Duke Energy is planned. Notifications to the Occupational Safety and Health Administration are planned. The NRC Senior Resident Inspector has been notified.
ENS 5075323 January 2015 11:13:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Non-Work Related On-Site Contractor FatalityAt approximately 0531 EST on January 23, 2015, the Brunswick Nuclear Plant main control room received an emergency call for a contract employee experiencing a non-work related medical issue. Site first responders were dispatched in conjunction with a request for off-site medical assistance. At approximately 0613 EST, the responding off-site paramedics determined that the efforts to revive the patient were unsuccessful. The individual was outside of the protected area (within the owner controlled area), and no radioactive material or contamination was involved. The cause of death has not been determined. This notification is being made in accordance with 10 CFR 50.72(b)(2)(xi) for situation related to the health of on-site personnel for which a notification to other government agencies is planned. The Occupational Safety and Health Administration (OSHA) will be notified. A press release is not planned at this time. The NRC Resident Inspector has been notified.
ENS 499933 April 2014 14:06:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Oil Spill on SiteAt 0730 EDT on April 3, 2014, it was determined that approximately 40 gallons of turbine lube oil (MOBIL DTE 732) had been released to the ground from the Unit 1 Turbine Building . The release occurred during the process of depressurizing the Unit 1 main generator. The source of the leak was identified and secured. The area of the oil spill has been contained, (and) no oil reached storm sewers or surrounding waters. Notification of the lube oil spill was made to the Underground Storage Tank Section of the North Carolina Division of Waste Management at 1006 hours EDT on April 3, 2014. This report is made in accordance with 10 CFR 50.72(b)(2)(xi) as an event related to protection of the environment for which notification to another government agency has been made. This event did not result in any adverse impact to the health and safety of the public. The safety significance of this event is minimal. There was no adverse impact to public health and safety. The area of the oil spill has been contained, no oil reached storm sewers or surrounding waters. Efforts are in progress to excavate the soil containing the lube oil. The source of the lube oil leak was identified and secured. Efforts are in progress to excavate the soil containing the lube oil. The licensee has notified the NRC Resident Inspector.
ENS 4876116 February 2013 19:03:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Onsite Contractor Fatality

At approximately 1318 hours (EST), the Main Control Room was notified of a contract employee slumped over on a fork lift inside the protected area. First Responders were dispatched to the scene. Off-site assistance was requested and arrived on-site at approximately 1330 hrs. CPR was administered by the First Responders and Paramedics. At 1403 hrs., the paramedics determined that the efforts to revive the patient were unsuccessful. The cause of death has not yet been determined. Unit 1 remains in Mode 1 at 100% power. Unit 2 remains in Mode 1 at 97% power. Power is limited due to end of cycle maximum core flow conditions. A press release is not planned at this time. The Occupational Safety and Health Administration (OSHA) will be notified of the fatality. This notification is being performed in accordance with 10CFR50.72(b)(2)(xi) for notification to an off-site government agency. The NRC Resident Inspector has been notified.

ENS 4821921 August 2012 23:30:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseDamaged Packaging Containing Radioactive MaterialIn accordance with 49 CFR 171.15, Duke Energy contacted the Department of Transportation National Response Center at 1930 (EDT) on August 21, 2012, Report Number 1021811, to inform them of a breach of a package containing a limited quantity of radioactive material. This breach was considered 'breakage' in accordance with Title 49 of the Code of Federal Regulations, Transportation. Although the packaging of the radioactive material was damaged during transportation, the radioactive material itself was not released, and the damage did not result in any external radioactive contamination or any radiation exposure. This event did not result in any adverse impact to the health and safety of the public. The State of North Carolina (NC) has also been notified of this event. Mecklenburg County, NC will be notified of this event. This event is being reported in accordance with 10 CFR 50.72(b)(2)(xi) as a result of the notification to another government agency. The safety significance of the breached package is minimal. The condition will be entered into the Corrective Action Program for evaluation. Corrective actions will be determined based on the results of this evaluation. The container held a torque wrench calibration test tool and cable. The tools had penetrated the container, but no other damage to the tools was noted. The tools will be normally processed for use at the station. The licensee has notified the NRC Resident Inspector.
ENS 4647310 December 2010 04:04:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification - Agencies Notified of Onsite Tritium Leak

On December 9, 2010, elevated levels of tritium were identified in water samples taken from in-leakage into the below ground elevation of the diesel generator building (i.e., through underground conduit penetrations). The tritium levels were determined to be in excess of the Nuclear Energy Institute (NEI) voluntary reporting criteria (i.e., 30,000 pCi/L for onsite groundwater, as specified in the Offsite Dose Calculation Manual). Further investigation determined that the likely source of the tritiated water is buried piping located west of the diesel generator building. The area of the leak is inside the plant protected area, well inside the site's property boundary. There is no indication that tritium has migrated into drinking water sources or has migrated off plant property. The leakage has not impacted plant reliability or the operability of any safety-related equipment. Corrective actions taken were to capture the in-leakage of water into the diesel generator building and route it through our normal permitted discharge paths. Additional water samples will continue to be obtained from site monitoring wells until the source of the leak is isolated. Efforts to identify and isolate the source of the leakage, and plans for excavation of the suspected area, are in-progress. The Brunswick plant has had an extensive groundwater protection monitoring program in place since 2007. This environmental sampling program consists of more than 100 monitoring wells which are routinely sampled. The following agencies will be updated on the status of the onsite tritium samples: City of Southport, Brunswick County, State Officials, NEI, INPO, and ANI. The Licensee has notified the NRC Resident Inspector.

On December 10, 2010, at 2124 hours (EST), the buried piping leak was stopped after isolating the Unit 1 Condensate Make-up line to the Main Condenser (i.e., from the Condensate Storage Tank (CST)). The elevated levels of tritium are confined to the area in close proximity to the identified buried pipe location, west of the diesel generator building, well within the site's property boundary. The perimeter monitoring wells to the plant's protected area and site boundary continue to be analyzed with no increase in tritium levels identified. Preparations for the excavation and repair of the Condensate Make-up line are in-progress. The following agencies will be provided an update of this event: City of Southport, Brunswick County, State Officials, NEI, INPO, and ANI. The initial safety significance of this event is minimal. The Brunswick Steam Electric Plant has not identified any health or safety risk to the public or onsite personnel. There is no impact to plant reliability or safety. The Licensee has notified the NRC Resident Inspector." R2DO (Moorman) notified.

ENS 448852 March 2009 14:15:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOnsite FatalityA contract employee associated with the Unit 2 refueling outage died this morning from an apparent heart attack. The death was not the result of any industrial safety issue and did not occur in a contaminated area. The individual was treated by the onsite first responders who administered CPR and AED (automatic external defibrillator). He was transported via ambulance to Dosher Memorial hospital where he was pronounced dead at 0915. OSHA is being notified of the event under the requirements of 29 CFR 1904. The licensee notified the NRC Resident Inspector.
ENS 440417 March 2008 15:30:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Tritium in Onsite Water SamplesBrunswick previously reported detection of tritium in excess of Nuclear Energy Institute (NEI) reporting criteria on June 13, 2007, i.e., EN 43420, as a result of water samples taken from an onsite shallow groundwater well (i.e., ESS 1BC) near the Storm Drain Stabilization Pond (SDSP). As previously indicated in EN 43420, the storm drain system routes tritiated water into the SDSP. Overflow from the Turbine Building air-wash system was identified as the primary contributor to the effluent of the storm drain system. The air-wash is no longer routed into the storm drains, which eliminated the main source of the tritium into the storm drain system. On March 6, 2008, an ongoing investigation determined that tritium levels in excess of NEI voluntary reporting criteria present in an onsite shallow groundwater well (i.e., ESS-2C), resulted from a condition not previously reported. Though elevated levels of tritium for each incident are most likely a result of the same source (i.e., effluent routed in the storm drain system,) subsequent investigations indicate that the storm drain piping may allow an inadvertent pathway to groundwater in close proximity to ESS-2C. The monitoring well ESS-2C is well within the boundaries of the site owner controlled area and there is no indication that tritium has migrated into any drinking water. The following agencies will be updated on the status of the onsite tritium samples and informed of the pending press release: State Officials, City of Southport, Brunswick County, NEI, INPO, and ANI. The Licensee notified the NRC Senior Resident Inspector.
ENS 436955 October 2007 22:47:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseSodium Hypochlorite Spill While Offloading a Tanker TruckAt 17:20 the Brunswick Station control room was notified of a sodium hypochlorite spill. Approximately 300 gallons of a 15% solution was released to the environment through a faulty hose fitting at a tanker truck before it could be isolated. The state is required to be notified of any hazardous chemical release greater than or equal to 100 pounds (64 gallons of a 15% sodium hypochlorite solution corresponds to 100 pounds). The North Carolina State Division of Water Quality was notified of the event at 18:47 as required by 40 CFR 355.40. The US Coast Guard (report number 850864) and Brunswick County were also notified of the event. Minimal impact to the environment. No impact to the health and safety of the public. No impact to plant safety or operations. The licensee notified the NRC Resident Inspector.
ENS 4342013 June 2007 12:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseNotifications Made for Detection of Tritium in Onsite Water SamplesOn May 8, 2007, measurable levels of tritium were identified in two manholes (i.e., MW-5 and MW-6) that route cable to a nearby meteorological tower. The manholes are located within the boundaries of the site owner controlled area. The manholes are also in close proximity to the Storm Drain Stabilization Pond (SDSP). The SDSP is an area of approximately 64 acres consisting of a non-lined perimeter dike of approximately 12 to 15 feet in height. The site storm drain system collects in the storm drain collector basin and is then pumped to the SDSP. The storm drain collector basin receives overflow from the Turbine Building air-wash system, which contains very small amounts of tritium. By plant design, the tritiated water of the SDSP is then pumped into the intake canal during monitored, controlled releases, compliant with all permits and regulations. As a result of ongoing investigations to determine the source and potential migration path of the tritium, an increased environmental sampling program was initiated. On June 13, 2007, tritium levels in excess of Nuclear Energy Institute (NEI) voluntary reporting criteria were confirmed to be present in new onsite shallow groundwater wells. Immediate corrective actions taken were to perform additional sampling of areas surrounding the stabilization pond and (to reconfigure) the Turbine Building air-wash system. The reconfiguration of the turbine building airwash system will be done to eliminate the designed pathway of water to the storm drain collection basin and to the storm drain stabilization pond. Additional corrective actions include installation of additional monitoring wells at various locations to determine groundwater flow and checking for the presence of tritium. There is no indication that tritium has migrated into drinking water sources and all offsite monitoring has resulted in non-detectable levels of tritium. It is believed that the likely source of the tritiated water is seepage from the SDSP. The following agencies will be updated on the status of the onsite tritium samples and informed of the pending press release: State Officials, City of Southport, Brunswick County, NEI, INPO, and ANI. The Licensee notified the NRC Resident inspector.
ENS 4095815 August 2004 17:22:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Made Due to Inadvertent Emergency Siren ActuationOn August 15, 2004, at approximately 1322 hours, the Brunswick site identified an inadvertent actuation of 1 offsite evacuation siren. There are a total of 36 sirens located in Brunswick and New Hanover Counties, NC. The siren in alarm is located in Brunswick County and was in an alarm condition from 1258 until 1341. In accordance with Brunswick operating procedures, the warning points (Brunswick County, New Hanover County and NC State EOC) have been notified of the inadvertent siren actuation. The remaining 35 sirens are operable. INITIAL SAFETY SIGNIFICANCE EVALUATION: Minimal safety significance. The loss of the single siren will not significantly impact the public notification system. The remaining sirens in both Brunswick and New Hanover Counties have been verified to be operable. CORRECTIVE ACTIONS: The siren was silenced at 13:41 EDT on August 15, 2004. The cause of the inadvertent actuation will be determined and the siren will be repaired. The (NRC) resident inspector has been notified.
ENS 4080210 June 2004 06:05:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Made Due to Inadvertent Emergency Siren ActuationOne emergency response siren (Number 10 Brunswick County) located in Southport, NC inadvertently actuated at 0205, apparently due to a lightning strike, and was silenced at 0312. The Brunswick County, New Hanover County and North Carolina State Emergency Operation Centers (EOC) were notified of the inadvertent actuation. The Brunswick County 911 Center received calls from local residents reporting the siren actuation. Initial Safety Significance Evaluation: Minimal safety significance. The loss of the single siren will not significantly impact the public notification system. The remaining sirens in both Brunswick and New Hanover Counties have been verified to be operable. Corrective Actions: The siren was silenced at 0312 on June 10, 2004. The cause of the inadvertent actuation will be determined and repaired. The NRC Resident Inspector was notified of this event by the licensee.
