10 CFR 55.25 requires in part, that the facility licensee notify the Commission within 30 days of discovery, that a licensed operator has been diagnosed with a permanent physical condition that adversely affects the performance of assigned operator job duties, so that the Commission can make a determination of the licensed operators medical fitness. Contrary to this requirement on March 20, 2003, the facility licensee identified that a licensed operator underwent a medical procedure in December 1998 that should have been reported to the NRC. This issue was of very low safety significance because upon review of additional information provided by the facility licensee, the NRC physician determined that a restriction would not have been required because the licensed operator would have been able to perform licensed responsibilities without impairment. This failure to report medical information to the NRC impacted the regulatory process, and therefore, is classified at
Severity Level IV.
