Description | Technical Specification 4.0.4 states, in part, that entry into an operational Mode shall not be made unless the surveillance requirement(s) .associated with the limiting condition for operation has been performed within the stated surveillance interval. Contrary to the above, while performing RPS matrix and trip path testing during the Unit 2 start-up on July 24, 2009, Dominion closed the TCSs and rendered the CEA drive system capable of CEA withdrawal without first demonstrating that the TCSs were operable. Dominion entered this issue into their corrective action program (CR442964) and is revising the surveillance procedure. This violation is of very low safety significance because the performance of the surveillance verified the operability of the TCSs. |
