NOTICE OF VIOLATION (Reply Required)

Indiana Michigan Power Company Donald C.

Cook Nuclear Power Plant Docket Nos. 50-315; 50-316 License Nos.

DRP-58; DRP-74 During an NRC inspection conducted on July 28,

1993, through September 10, 1993, violations of NRC requirements were identified.

In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1991), the violations are listed below:

a.

Unit 1

and 2 Technical Specification 6.8. la requires, in part, tha't written procedures be implemented covering the activities referenced in Appendix "A" of Regulatory Guide 1.33, Rev.

2, February

1978, which includes procedures for..."Inspection of Reactor Coolant System Pressure Boundary..."

Licensee procedure "Plant guality Control Program,"

PMI-7090, Revision 5, dated February 24, 1992, required that "inspection sign-offs in a procedure... shall be identified and space provided for signature of the inspector and date.

Inspections performed shall include... for safety related items... tightness of connections and fastenings for pressure boundary componerts."

Contrary to the above, Attachment No.

10 of licensee procedure "Reactor Reassembly,"

OHP 4050.FHP.004 for Units 1

and 2, Revision 1, did not include inspection sign-offs for verifying the tightening of the fasteners that provide the pressure boundary

-"or the upper and lower conoseals.

This is a Severity Level IV violation (Supplement I).

b.

Title 10 of the Code of Federal Regulations, Part 50, Appendix B, Criterion V, "Instructions, Procedures, and Drawings," requires that activities affecting quality shall be prescribed by documented procedures of a type appropriate to the circumstances and shall be accomplished in accordance with these procedures.

Procedures shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished.

I Contrary to the above, licensee procedure "Motor Driven Auxiliary Feed pump Maintenance,"

    • 12MHP5021.056.001, Revision 5, did not contain instructions to install the pump thrust bearings onto the shaft in the "back-to-back" position as specified by the Ingersoll-Rand pump manual "Auxiliary Feedwater Pump 3-HMTA-8."

This is a Severity Level IV violation (Supplement I).

Notice of Violation Pursuant to the provisions of 10 CFR 2.201, Indiana Michigan Power Company is hereby required to sub~it a written statement or explanation to the U.S.

Nuclear Regulatory Commission, ATTN: Oocument Control

Oesk, Washington, O.C.

20555 with a copy to the Regional Administrator, Region III, 799 Roosevelt

Road, Glen Ellyn, Illinois, 60137, and a copy to the NRC Resident Inspector at the Oonald C.

Cook Nuclear Power Plant within 30 days of the date of the letter transmitting this Notice of Violation (Notice).

This reply should be clearly marked as a "Reply to a Notice Of Violation" and should include for each violation: (1) the reason for'he violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results

achieved, (3) the corrective steps that will be taken to avoid further violations,.and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order or demand for information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken.

Where good cause is shown. consideration will be given to extending the response time.
