discussion of our findings with A. A. Blind and others of your staff at the

conclusion of the inspection.

Areas

examined during the inspection

are identified in the report.

Within

these

areas,

the inspection consisted

of selective

examinations of procedures

and representative

records,

interviews with personnel,

and observation of

activities in progress.

Based

on the results of this inspection,

certain of your activities appeared

to be in violation of NRC requirements,

as specified

in the enclosed

Notice of

Violation (Notice).

The violation regarding

the lack of inspection

hold

points for ensuring

proper tightness

of fasteners

used during reassembly

of

the conoseal

is of concern

because it could lead to the conoseals

being

reassembled

improperly.

Your response

to this violation should also address

the root cause

and corrective action for the document control problem

associated

with this issue.

The violation regarding

the lack of specificity in the procedure for

installation of the thrust bearings for the auxiliary feedwater

(AFW) pump is

of particular concern.

It contributed to the incorrect installation of these

bearings

in your Unit 2 motor-driven

AFW pump

and consequent

accelerated

bearing wear.

These

same bearings

had twice previously been

found damaged,

as

discussed

in our recent

Inspection

Report

No. 50-316/93016

(DRP).

The recent

report transmitted

a Notice of Violation for failure to vigorously pursue

auxiliary feedwater deficiencies.

In addition,

our inspectors

have repeatedly

emphasized

attentiveness

to AFW.

Thus,

our discovery during this inspection

period (during

a tour by the

NRC Regional Administrator) 'that oil level

on one

Unit

2

AFW pump was below pump vendor acceptance

levels,

was cause for

additional

concern.

In responding

to the

AFW violation, please

address

your

plans to enhance

attention to the condition of this vital safety equipment.
